Lichen sclerosus of the vulva is a common, but poorly studied disease. We assessed the level of transcriptional activity of APAF1, BAX, BCL2, BIRC5, CCND1, DAPK1, MCL1, and MYC genes encoding products that control apoptosis in the samples of tissues affected by vulvar lichen sclerosus and adjacent control tissues (n=24). Analysis of transcriptional activity was performed by real-time PCR using specific primers and SYBR Green intercalating dye. After the total group was divided by the presence of the concomitant gynecological diseases, a significant increase in the transcriptional activity of the CCND1 gene was revealed in patients with concomitant uterine fibroids. This may indicate the possible role of the activation of mitosis during tumor initiation.

The number of viral infection cases in the Department of Gynecology and Obstetrics has tended to increase over last few years. Viruses form herpesvirus and cytomegalovirus families are associated with an increased risk for recurrent pregnancy loss. Photodynamic therapy (PDT) is a promising new approach to treat viral infections in which viral particles are inactivated. It exhibits great therapeutic potential, particularly among this group of patients. This study examined the use of PDT to treat herpesvirus infection (HVI) using an in vitro model. In this study, we used the Vero Ñell lineage as a suitable model of HVI, strains of HSV-1 (strain VR-3) and HSV-2 (strain MS) obtained from The National Virus Collection (London, UK), the photosensitizer Fotoditazine (Veta-Grand, Russia), an AFS physiotherapeutic device (Polironic Corporation, Russia). Laser light irradiation and the photosensitizer had different cytotoxic effects on the Vero cell cultures depending on the doses used. The optimal laser light and photosensitizer doses were determined. PDT had an antiviral effect on an in vitro model of HVI in cell culture. PDT has been shown to be effective treatment for HVI in vitro, leading to a reliable decrease of viral titer.

A complex simultaneous assessment of the microbiota (the aerobic and anaerobic links as well as the parietal and lumen components) of the gutter, vagina, and intestines of women with a pathological pregnancy was performed. In 30% of women the study revealed system dysbiotic changes. Local immune reactivity was decreased, which may be considered a provoking factor. In conclusion, complex microbiological examination makes it possible to objectivize the picture of the pathological process and its outcome. A criterion for the administration of correcting measures is offered.

OBJECTIVE: To optimize methods of surgery in patients with malformations of the genitalia. METHODS: Comparative evaluation of effectiveness of reconstructive plastic operations for malformations of uterus and vagina, performed by conventional methods or via laparoscopy. RESULTS: Laparoscopy permits identification of the pelvic peritoneum plus opening it and using the most mobile portion to create the vaginal fornix. In patients with vaginal and cervical aplasia and non-communicating functional rudimentary uterus, laparoscopy assists hysterectomy performed by the vaginal approach, as well as the last step of colpopoiesis. CONCLUSIONS: Laparoscopy significantly facilitates the procedure, reduces operating time and risks, and makes the operation available to the wide range of surgeons skilled in laparoscopy.

Clinical status, the major parameters characterizing the course of surgery, early postoperative period, immediate and late results of treatment are analyzed in 261 patients. Besides the routine methods, endoscopic methods (laparoscopy and hysteroresectoscopy) were used at the principal stages of the operation in 97 patients, and in 18 more patients CO2 laser was used for metroplasty. Use of present-day equipment and methods facilitates surgical repair of developmental defects of the genitals, reduces the duration of the operation, decreases blood loss, shortens hospital stay, appreciably improves the efficacy of surgical treatment, and places the operations for correction of developmental defects among those available for surgeons who mastered the endoscopic methods. Hence, modern endoscopic methods may be effectively used for both the differential diagnosis of developmental defects of the genitals and for surgical repair thereof.

Follow-up and therapy of 136 patients with nonspecific salpingo-oophoritis evidence the efficacy of acupuncture included in a complex of therapeutic measures for the treatment of this condition, as compared to traditional therapeutic modalities. Acupuncture therapy is conductive to shortening in-hospital therapy, to full-value normalization of the body immunobiologic defense and of the autonomic nervous system, and to a reduction of the incidence of the condition recurrences.
